Air medical providers around the country fly a relatively consistent set of aircraft families. Turboprops carry many regional fixed-wing missions because they combine useful range with access to smaller airports. Light jets cover longer domestic routes and international work. Helicopters cover short distances and landing sites without runways.

What roles do fixed-wing aircraft and helicopters play?

Fixed-wing aircraft and helicopters solve different problems. A helicopter can land at an accident scene, a rural road, or a hospital helipad, which makes it the standard tool for scene response and short transfers between nearby facilities. Its range and cabin space are limited, so it is not the typical choice for a long route.

A fixed-wing air ambulance needs an airport at each end, which means ground ambulances normally complete the trip between the facilities and the airports. In exchange, a fixed-wing aircraft covers far greater distance, flies in a wider range of conditions, and offers a larger, pressurized cabin for the patient, medical crew, equipment, and often a family member.

What does a medical configuration include?

The same aircraft model can fly as a charter airplane, a cargo platform, or an air ambulance. The difference is the medical configuration installed in the cabin. A typical air ambulance configuration includes:

  • A stretcher and loading system that secures the patient and allows loading through the aircraft door.
  • Medical oxygen with the capacity planned for the full mission.
  • A transport ventilator for patients who need breathing support.
  • Cardiac monitoring with defibrillation capability.
  • Medication and infusion pumps for continuous IV medications during flight.
  • A neonatal isolette for missions involving newborn patients, when required.

Who owns and operates these aircraft?

Air ambulance aircraft in the United States are owned and operated by independent air carriers. On-demand air ambulance flights are operated under Federal Aviation Administration Part 135 authority, and the certificated carrier holds operational control of the flight: the aircraft, the pilots, maintenance, dispatch, and the final go or no-go decision.
